Job summary

Advantmed in-home wellness and risk‑adjustment assessments is hiring a licensed Nurse Practitioner for 1099 PRN work. You will conduct structured visits with autonomy and strong operational support, earning around $110 per completed assessment plus mileage.

Schedule is flexible weekdays with no evenings or weekends, and daily travel within a 55‑mile radius is expected. Ideal candidates are experienced NPs with EMR proficiency, a focus on preventive care, and the ability to work independently

Qualifications

• Active NP license in the applicable state.
• 3+ years of patient care experience preferred.

Responsibilities

• Conduct in-home Annual Wellness Visits and chronic condition assessments.
• Perform histories, exams, vitals, medication reviews, and screenings.
• Educate members on preventive care and chronic disease management.
• Document visits in the EMR to support value-based care and risk adjustment.
