Optum HouseCalls in Hartford, CT is seeking a Nurse Practitioner or Physician Assistant to perform in-home health assessments for Medicare Advantage and other health plan members. This field-based, non-prescribing role focuses on preventive care, patient education, gap closure, and collaboration with primary care providers to improve health outcomes.

Compensation includes $40,000 student loan repayment or a $20,000 sign-on bonus, plus a range of benefits.

Qualifications

• NP/PA certification requirements (ANCC/AANP or NCCPA) and state licensure are required.
• Unrestricted licensure in practicing state and valid driver's license required.
• Bilingual in Spanish is required.

Responsibilities

• Conduct comprehensive in-home health assessments for Medicare Advantage and other plans.
• Educate patients and support preventive care initiatives.
• Partner with PCPs to address care gaps and care coordination needs.
• Identify social determinants of health and connect patients with resources.
• Independent, field-based role with interdisciplinary clinical support.
