Job summary

Optum in Hartford & New Haven, CT is seeking a Nurse Practitioner or Physician Assistant to perform annual in-home health assessments for Medicare Advantage and other health plan members. This field-based role focuses on preventive care, patient education, gap closure, and collaboration with primary care providers to improve health outcomes.

The position offers flexible scheduling, no on-call, weekends, or holidays, and compensation incentives including student loan repayment or a sign-on bonus,

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care for NPs.
• Active NCCPA certification and state licensure for PAs.
• Active and unrestricted medical licensure in practicing state without disciplinary actions.
• Active and unrestricted driver's license with access to reliable transportation for home visits.
• Spanish-speaking ability is required.

Responsibilities

• Conduct in-home health assessments for Medicare Advantage and other health plan members.
• Educate patients and support preventive care initiatives.
• Collaborate with PCPs to address care gaps and coordinate care.
• Identify social determinants of health and connect patients with resources.
• Operate in an independent field-based role with interdisciplinary clinical support.
