Job summary

OptumCare is seeking a Nurse Practitioner or Physician Assistant to perform annual in-home health assessments for Medicare Advantage and other health plan members. This field-based role emphasizes preventive care, patient education, and collaboration with primary care providers to improve outcomes.

The position offers a $20,000 sign-on bonus or $40,000 student loan repayment, flexible scheduling, and no on-call duties.

Qualifications

• For NP: Active ANCC/AANP certification in Family, Adult, Ger or Adult-Gerontology Primary Care
• For PA: Current NCCPA certification and state licensure
• Active, unrestricted licensure in practicing state without disciplinary actions
• Driver’s license and access to reliable transportation for home visits

Responsibilities

• Conduct comprehensive in-home health assessments for Medicare Advantage and other health plan members
• Identify and close gaps in care through collaboration with primary care providers
• Educate patients on disease management, medications, and preventive care
• Address social determinants of health and connect members with resources
