Job summary

Monogram Health is seeking Advanced Practice Providers (NPs or PAs) to deliver in-home care to patients with multiple chronic conditions. You’ll work within an integrated care team, delivering longitudinal care in patients’ homes and through telehealth, with a value-based, patient-centered approach.

Responsibilities include comprehensive assessments, education, and coordination with PCPs. Travel to patient homes is required; a robust benefits package and competitive base salary are offered.

Qualifications

• Active and unrestricted RN and NP license, or PA license.
• Board certification per license type.
• DEA certificate active or eligible.

Responsibilities

• Provide in-home patient care to complex patients.
• Perform comprehensive assessments and annual wellness visits.
• Educate patients and caregivers on disease management and preventive care.
• Coordinate care with PCPs and specialists.
• Document accurately and timely.
