Qualifications

• Provide advanced clinical care to homebound patients with multiple chronic conditions.
• Deliver comprehensive assessments, wellness exams, and post-discharge visits.
• Maintain compliance with state/federal regulations and documentation standards.

Responsibilities

• Provide evidence-based, patient-focused care in the patient’s home.
• Perform comprehensive assessments and follow-up visits.
• Educate patients and caregivers on disease management and medications.
• Participate in virtual care team meetings and quality activities.
• Coordinate care with PCPs, specialists, and ancillary services.
