Job summary
Monogram Health Professional Services is seeking Advanced Practice Providers (NPs or PAs) to deliver high-quality in-home care to patients with multiple chronic conditions across various settings using a value-based care model.

Qualifications
- Active and unrestricted RN and NP license, or Physician Assistant license.
- Board certification (NP: ANCC/AANP | PA: NCCPA)
- DEA certificate required or eligibility
- Minimum 2 years of direct patient care
- Travel to patient homes with reliable transportation
- Experience with EHR systems (Athena, Salesforce, Epic)

Responsibilities
- Provide evidence-based, patient-focused care in patients' homes.
- Perform comprehensive assessments and wellness exams.
- Educate patients and caregivers on disease management and preventive care.
- Participate in virtual integrated care team meetings.
- Ensure compliance with state boards and clinical best practices.
- May practice in SNFs, ALFs, hospitals, and other settings as applicable.
- Coordinate care with PCPs, specialists, and ancillary services.
