You will join an interdisciplinary team serving patients across multiple states and markets, delivering care that emphasizes comfort, dignity, and home-based solutions.

Qualifications

• Licensed as a NP or PA in the State where HarmonyCares Medical Group office located.
• Federal DEA License/eligibility.
• Geriatric training/experience preferred.
• Active CPR/BLS Certification.
• Board Certification.
• Must maintain a valid driver's license and good driving record.

Responsibilities

• The Advance Practice Providers we seek are those who have directly related experience in home health, geriatrics, hospice and primary care.
