HarmonyCares is seeking a Nurse Practitioner to deliver annual in-home risk assessments and telehealth visits within the scope of practice. The role involves patient evaluations, history taking, diagnoses, and documentation in line with NCQA standards.

The ideal candidate holds an active NP license, board certification, BLS, and strong EHR skills, with experience in home-based care and collaboration with physicians and the care team.

Competenze

• Active nurse practitioner license in coverage states
• Board certification in ANCC, AANP or NCCPA
• Active BLS Certification
• Current enrollment in Medicare/Medicaid
• Strong EHR skills and documentation

Mansioni

• Conduct comprehensive in-home health risk assessments and identify active and chronic disease conditions
• Take patient history, perform examinations, determine diagnoses
• Provide written documentation of patient visits per NCQA standards
• Measure vital signs and coordinate with PCP as needed
• Place case management referrals and communicate with care team
• Communicate with patients, caregivers, agency nurses, and other providers
• Maintain patient confidentiality and OSHA compliance
• Attend meetings and participate in professional development
• Be flexible to perform other duties as assigned
