Job summary

Monogram Health is seeking compassionate Advanced Practice Providers (NP/PA) to deliver high-quality in-home care to patients with multiple chronic conditions. The role involves working within an integrated care team, providing home visits and telehealth, and contributing to a value-based care model.

Travel to Austin, New Braunfels, and San Antonio as needed, with 3–4 home visits weekly and telehealth options.

Qualifications

• Active and unrestricted RN and NP license, or Physician Assistant license.
• Board certification (NP: ANCC/AANP | PA: NCCPA).
• Current and unrestricted DEA certificate or eligibility to obtain.
• Minimum of two years of direct patient care experience.

Responsibilities

• Provide evidence-based, patient-focused care in the patient’s home.
• Perform comprehensive assessments and wellness visits.
• Educate patients and caregivers on disease management and medications.
• Coordinate care with PCPs, specialists, and ancillary services.
• Complete documentation accurately and timely.
