Job summary

Monogram Health is seeking a Float Advanced Practice Provider (NP/PA) to deliver in-home care to patients with multiple chronic conditions in Austin, New Braunfels and San Antonio, with a clustered schedule and telehealth integration.

The role involves comprehensive patient assessments, care coordination, and collaboration with an interdisciplinary team to reduce hospitalizations and improve outcomes. Travel and flexibility are required, with competitive compensation and mileage reimbursement.

Qualifications

• Active and unrestricted RN or NP license, or PA license.
• Board certification (NP: ANCC/AANP | PA: NCCPA).
• Current DEA certificate or eligibility to obtain.
• Minimum of two years direct patient care experience.
• Field-based role requiring travel to patient homes; valid driver’s license and auto insurance.
• BLS certification (onboarding).
• Knowledge of federal/state regulations (HIPAA/OSHA).
• Experience with EHR systems (e.g. Athena, Salesforce, Epic).
• Previous experience with annual wellness or comprehensive visits preferred.
• Background in managed care, population health, or value-based care preferred.

Responsibilities

• Provide evidence-based, patient-focused care in the patient’s home to reduce hospitalizations and improve quality of life.
• Perform comprehensive evaluations, wellness exams, follow-ups, urgent visits, and post-discharge visits.
• Educate patients and caregivers on disease management, preventive care, medications, and testing.
• Participate in recurring virtual integrated care team meetings.
• Engage in quality assurance activities to ensure regulatory compliance and best practices.
• May practice in SNFs, ALFs, hospitals, and other clinical settings as needed.
• Perform point-of-care testing (A1C, Spirometry), prescribe meds, order tests, and coordinate with Monogram physicians.
• Coordinate care with PCPs, specialists, and ancillary services.
• Complete documentation accurately and timely.

Education

RN/NP/PA license

Board certification (NP: ANCC/AANP | PA: NCCPA)

DEA certificate

Direct patient care experience (2+ years)

Travel readiness and valid driver’s license

BLS certification

HIPAA/OSHA knowledge

EHR experience (Athena, Salesforce, Epic)

Annual wellness/comprehensive visits experience (preferred)

Managed care/population health background (preferred)
