Job summary

Monogram Health is seeking compassionate Advanced Practice Providers (NPs or PAs) to deliver in-home care to patients with multiple chronic conditions. You will join an integrated care team and provide continuity of care in patients' homes and via telehealth.

As part of a value-based, patient-centered model, you will conduct comprehensive assessments, coordinate care with PCPs and specialists, perform point-of-care testing, and support education and disease management.

Qualifications

• Active NP or PA license with board certification.
• Current and unrestricted DEA certificate or eligibility.
• Minimum of two years of direct patient care experience.

Responsibilities

• Provide evidence-based, patient-focused care in the patient’s home.
• Perform comprehensive assessments and follow-up visits.
• Educate patients and caregivers on disease management and preventive care.
• Coordinate care with PCPs, specialists, and ancillary services.
• Complete documentation accurately and timely.
• May practice in SNFs, ALFs, hospitals, and other settings as applicable.
