Job summary

Monogram Health seeks compassionate Advanced Practice Providers (NPs or PAs) to deliver in-home care for patients with multiple chronic conditions across various settings, including telehealth. You’ll join an integrated care team and help establish long-term relationships in a value-based model.

Role involves travel to patient homes in Austin, New Braunfels, and San Antonio, with a clustered schedule and 3–4 patient visits per week.

Qualifications

• Active and unrestricted RN and NP license, or Physician Assistant license
• Board certification (NP: ANCC/AANP | PA: NCCPA)
• Current and unrestricted DEA certificate, or eligibility to obtain
• Minimum of two (2) years of direct patient care experience
• Field-based role requiring travel to patient homes, reliable transportation, valid driver’s license, and auto insurance
• Demonstrated initiative and independent, critical thinking, and decision-making skills
• Knowledge of and compliance with federal/state regulations (HIPAA, OSHA)
• Prior EHR experience (Athena, Salesforce, Epic)
• Background in managed care/population health/value-based care preferred

Responsibilities

• Provide in-home, patient-focused care to reduce hospitalizations and improve quality of life
• Perform comprehensive assessments including initial evaluations and annual wellness exams
• Educate patients and caregivers on disease management and medications
• Participate in virtual integrated care team meetings
• Engage in quality assurance activities and documentation reviews
• May practice in SNFs, ALFs, hospitals, and other settings as applicable
• Perform point-of-care testing and coordinate with Monogram physicians
• Coordinate care with PCPs, specialists, and ancillary services
• Complete timely and accurate documentation
