Job summary

Monogram Health, Inc. seeks a Float Advanced Practice Provider (NP/PA) to deliver in-home care to patients with multiple chronic conditions across Austin, New Braunfels, and San Antonio.

You’ll work with an integrated care team, visit homes 3–4 days/week, and include telehealth visits, with no on-call or overnights. The role offers a competitive base salary, performance-based bonuses, and mileage reimbursement.

Qualifications

• Active and unrestricted RN and NP license, or Physician Assistant license.
• Board certification (NP: ANCC/AANP | PA: NCCPA).
• Current and unrestricted DEA certificate, or eligibility to obtain
• Minimum of two years direct patient care experience
• Field-based role requiring travel to patient homes, reliable transportation, valid driver’s license
• Knowledge of federal/state regulations (HIPAA, OSHA)
• Prior experience with EHR systems (Athena, Salesforce, Epic)
• Previous experience conducting annual wellness or comprehensive visits preferred
• Background in managed care, population health, or value-based care preferred

Responsibilities

• Provide evidence-based, patient-focused care in the patient’s home.
• Perform comprehensive evaluations and follow-ups.
• Educate patients and caregivers on disease management and medications.
• Participate in recurring virtual care team meetings.
• Engage in quality assurance and chart audits.
• May work in SNFs, ALFs, hospitals, and other clinical settings.
• Coordinate care with PCPs and specialists.
• Complete timely documentation.
