UnitedHealth Group’s Optum HouseCalls is seeking an Advanced Practice Clinician (NP/PA) to perform in-home health assessments for Medicare Advantage members and other plan members. This field-based, non-prescribing role emphasizes education, gap closure, and collaboration with primary care providers to improve health outcomes.

The role offers flexible scheduling, no on-call, and no weekend or holiday requirements, with compensation reflecting base pay plus bonus eligibility.

Qualifikationen

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care (NPs).
• Current NCCPA certification and state licensure (PAs).
• Active, unrestricted licensure in the practicing state and ability to obtain by start date.

Aufgaben

• Conduct comprehensive in-home assessments for Medicare Advantage and other plan members.
• Identify and document diagnoses for care management and treatment planning.
• Communicate findings to members' PCPs to address gaps in care.
• Recognize urgent situations and intervene appropriately.
• Educate members on disease processes, medications, and adherence; address social determinants of health and provide referrals.
