Job summary

Optum House Calls is seeking an Advanced Practice Clinician (NP or PA) to perform in-home Medicare assessments. This field-based role focuses on education, gap closure, and collaboration with primary care providers to improve health outcomes.

No prescribing responsibilities in this role; licensure and certification required, with travel to patient homes. Flexible schedule 8am–8pm across a 7-day week, supportive team environment, and opportunities for career growth.

Qualifications

• Active ANCC/AANP certification for NP (Family, Adult, Gerontic, or Primary Care)
• NCCPA certification for PA and current state licensure
• Active, unrestricted licensure in the practicing state
• Driver's license with reliable transportation for home visits
• Ability to navigate varied home environments during assessments
• 1+ years of clinical experience preferred

Responsibilities

• Conduct comprehensive in-home assessments including history, meds, vitals, and physical exam
• Identify and document diagnoses for care management and treatment planning
• Communicate findings to members' PCPs to address gaps in care
• Recognize urgent/emergent situations and intervene appropriately
• Educate members on diseases, medications, and adherence
• Address social determinants of health and provide referrals as needed
