Optum Home & Community Care, based in NM, seeks an in-home health clinician to perform preventive wellness evaluations for Medicare Advantage members. You will identify care gaps and collaborate with primary care providers to improve outcomes.

This field-based role offers scheduling flexibility between 8am and 8pm over a 7-day week, with no on-call, weekends, or holidays required. Base pay may include incentives, and bilingual clinicians may receive additional compensation.

Formación

• NP or PA professional licensure and certification required.
• Active, unrestricted state license with no disciplinary actions.
• Active driver's license and reliable transportation for home visits.
• Ability to perform assessments in varied home environments.

Responsabilidades

• Conduct comprehensive in-home preventive health assessments for Medicare Advantage and other members.
• Identify care gaps and coordinate with primary care providers to close them.
• Provide scheduling flexibility with home visits during 8am–8pm across a 7-day week.
• Support language needs by utilizing additional language skills when applicable.
