UnitedHealth Group's Optum Home & Community Care is seeking an Advanced Practice Clinician to perform in-home health assessments for Medicare Advantage members. This field-based role emphasizes education, gap closure, and collaboration with primary care providers to improve outcomes without prescribing.

The position offers flexible scheduling, 8am–8pm coverage over a 7-day week, no on-call, weekends, or holidays, and a compensation package with base pay plus bonus.

Qualifikationen

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult‑Gerontology Primary Care.
• Current NCCPA certification for PAs and state licensure.
• Unrestricted license to practice in the state of start date.

Aufgaben

• Conduct in-home assessments for Medicare Advantage and other members.
• Documentation of diagnoses and care planning in collaboration with PCPs.
• Educate members on disease processes, medications, and adherence.
• Identify urgent situations and intervene or refer as needed.
• Address social determinants of health and provide referrals.
