Signify Health, part of CVS Health, is seeking a Per Diem Clinician (NP or PA) to provide in-home health evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.

The visit lasts about 45 minutes and includes medication/history review, a physical evaluation, and optional diagnostic tests if ordered by the health plan. Travel within licensed states and virtual visits are required as needed.

Formación

• Active, unrestricted licenses in coverage areas.
• Board certification is required.
• Multi-state licenses are preferred.
