Job summary

Signify Health, part of CVS Health, is seeking a Per Diem Clinician (NP or PA) to perform in-home health evaluations. You will conduct comprehensive visits, review medications and histories, and perform physical assessments in members' homes.

The role offers part-time hours, travel within licensed states, and opportunities to perform virtual visits. Board-certified NPs or PAs with active licenses are preferred; multi-state licenses are a plus.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification in NP or PA (required).
• Multi-state licenses (preferred).
