Job summary

Signify Health, part of CVS Health, seeks a Per Diem Clinician (NP or PA) to provide In-Home Health Evaluations, visiting members at their homes and helping bridge gaps in care. Visits last about 45 minutes and include medication/history review and a clinical evaluation.

You will travel locally within licensed states, conduct virtual visits as needed, and use an iPad with a streamlined clinical workflow. This is part-time with competitive pay and benefits for eligible staff.

Qualifications

• Active, unrestricted license in the coverage area.
• Board certification is required.
• Multi-state licenses are preferred.

Responsibilities

• Conduct in-home health evaluations for members.
• Engage with patients to address health questions and bridge gaps in care.
• Utilize an iPad and clinical workflow; travel to homes as needed.
