Job summary

Signify Health, part of CVS Health, is seeking a Per Diem Clinician (NP or PA) to provide in-home health evaluations. You will engage with members in their homes, complete assessments, review medications and medical history, and support bridging gaps in care.

The role requires active license, board certification, and willingness to travel locally; part-time, about 12 hours weekly, with pay range $46.03–$99.14 per hour and comprehensive benefits.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).

• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.)
