Job summary

Signify Health, part of CVS Health, is seeking a Per Diem Clinician (NP/PA) to provide in-home health evaluations for members in their homes. You will use an iPad with a clinical workflow, review medications and history, perform physicals, and coordinate with health plans for any required tests.

The role offers part-time hours, with a typical weekly commitment of 12 hours. Per visit compensation includes an hourly base rate plus incentives; malpractice insurance is provided during Signify Health

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification required.
• Multi-state licenses preferred.

Responsibilities

• Provide in-home health evaluations with 45-minute visits.
• Bridge gaps in care by connecting patients to appropriate services.
• Travel locally and within licensed states as needed; conduct virtual visits when required.
• Perform Diagnostic & Preventive Services as indicated by health plans.
