Job summary

Signify Health, part of CVS Health, is seeking a Per Diem Clinician (NP or PA) to perform in-home health evaluations. Visits are ~45 minutes and include history review, physical exam, and plan of care discussions with members in their homes.

You will travel locally within licensed states, partner with health plans, and use an iPad with streamlined workflows. This part-time role offers competitive pay and benefits for eligible hours, with malpractice coverage provided.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).

Responsibilities

• Conduct in-home health evaluations for members.
• Perform 45-minute visits including medication and medical history reviews and physical evaluations.
• Coordinate with health plans to bridge gaps in care and connect to the right services.
• Travel locally to licensed states and conduct virtual visits as needed.
