Job summary

Optum HouseCalls is hiring a Nurse Practitioner or Physician Assistant to perform annual in-home health assessments for Medicare Advantage members. This is a non-prescribing, field-based role focused on preventive care, patient education, and coordination with primary care providers to close care gaps.

Qualifications

• RN/NP/PA certification required with valid licensure and driving eligibility.
• Experience in family practice, geriatrics, or home health preferred.
• Ability to travel 85% and work in patients' homes using preventive care approach.

Responsibilities

• Conduct comprehensive in-home health assessments for Medicare Advantage members.
• Review medical history, medications, and perform exams and screenings.
• Identify care gaps and coordinate with primary care providers to improve outcomes.
• Educate members on disease management and preventive care; address social determinants of health.
