Sampson and Bladen Counties, NC, Optum invites qualified NP or PA to perform in-home health assessments for Medicare Advantage members. This field-based role emphasizes preventive care, patient education, and close collaboration with primary care teams.

The ideal candidate will hold an active NP or PA credential, possess unrestricted licensure, and have reliable transportation for home visits. Excellent work-life balance and no on-call requirements are offered.

Formación

• Active and unrestricted licensure in the practicing state.
• Certification either as NP (ANCC/AANP) or PA (NCCPA).
• Access to reliable transportation and a valid driver's license.

Responsabilidades

• Conduct in-home health assessments for Medicare Advantage and health plan members.
• Educate patients on disease management, medications, and preventive care.
• Identify and close care gaps; address social determinants of health.
• Collaborate with an interdisciplinary care team; support primary care providers.
