Job summary

OptumCare is seeking a Nurse Practitioner or Physician Assistant to perform annual in-home health assessments for Medicare Advantage and other health plan members in Hyde and Washington Counties, NC. This field-based, non-prescribing role emphasizes preventive care, patient education, and collaboration with primary care providers.

Key duties include comprehensive home visits, closing care gaps, educating patients on management and prevention, and addressing social determinants of health.

Qualifications

• Active NP/PA certification requirements: NP with ANCC or AANP or PA with NCCPA.
• State licensure current and unrestricted; ability to obtain licensure by start date if required.
• Driver's license and reliable transportation for home visits.
• Ability to navigate varied home environments and perform physical assessments.

Responsibilities
