Job summary

OptumCare is seeking a Nurse Practitioner or Physician Assistant to perform annual in-home health assessments for Medicare Advantage and other plan members in Duplin and Sampson Counties, NC. This field-based, non-prescribing role focuses on preventive care and education, with collaboration from a multidisciplinary team to improve health outcomes.

The position offers a $20,000 sign-on bonus or $40,000 student loan repayment, flexible scheduling, and no on-call, weekends, or holidays.

Qualifications

• Active ANCC or AANP certification for NP or NCCPA certification and state licensure for PA
• Unrestricted licensure in practicing state; eligibility to obtain by start date
• Driver's license and reliable transportation for home visits
• Ability to navigate varied home environments and perform assessments

Responsibilities

• Conduct comprehensive in-home health assessments for members
• Identify and close gaps in care and coordinate with primary care providers
• Educate patients on disease management, medications, and preventive care
• Address social determinants of health and connect members with resources
