Job summary

OptumCare is seeking a Nurse Practitioner or Physician Assistant to perform annual in-home health assessments for Medicare Advantage members in Sampson and Bladen Counties, NC. This field-based role emphasizes preventive care and collaboration with primary care providers.

You will educate patients on disease management, medications, and preventive care while addressing social determinants of health. The position is independent, with support from an interdisciplinary team and flexible scheduling.

Qualifications

• NP/PA certifications required (family/adult/geriatric primary care).
• Active, unrestricted licensure in practicing state or ability to obtain by start date.
• Valid driver's license and reliable transportation for home visits.

Responsibilities

• Conduct comprehensive in-home health assessments.
• Identify and close care gaps.
