Job summary

Optum HouseCalls is seeking a Nurse Practitioner or Physician Assistant to perform annual in-home health assessments for Medicare Advantage members in Craven & Jones Counties, NC. This field-based role focuses on preventive care, patient education, and collaboration with primary care teams to improve health outcomes.

Requirements include active NP/PA certification, state licensure, and a valid driver’s license with reliable transportation.

Qualifications

• Must hold active licensure in the practicing state; no disciplinary actions.
• Active and unrestricted driver’s license; reliable transportation for home visits.
• Ability to navigate varied home environments and perform assessments as required.

Responsibilities

• Conduct comprehensive in-home health assessments for Medicare Advantage and other members.
• Educate patients on disease management, medications, and preventive care.
• Identify and close gaps in care; collaborate with primary care providers and interdisciplinary team.
• Address social determinants of health and connect members with resources.
