Job summary

Matrix Medical Network is seeking a licensed NP to perform comprehensive in-home health assessments for adults and seniors. You will review medical history, medications, and social factors, and coordinate with care teams to close gaps in care.

Visits occur at members' homes or SNFs, with flexible scheduling and potential weekend shifts. Responsibilities include conducting assessments, ordering tests, and providing education and follow-up.

Qualifications

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority if needed.
• AANP, AACN, or ANCC board certification in an adult-gerontology, family, or acute care specialty.
• Current BLS, ACLS or CPR certification.
• 1 year of NP experience.
• Travel up to 80-90% including overnight travel.
• Valid state driver’s license and automobile insurance.
• Strong computer and EMR skills.

Responsibilities

• Conduct adult and geriatric assessments, document findings per Matrix guidelines.
• Administer and order point-of-care and lab testing as needed.
• Coordinate with physicians, case managers, social workers, and caregivers.
• Educate patients and follow up with PCP for continuity of care.
• Deliver services at the member's home or SNF locations.
