Job summary

Hueman is actively hiring Nurse Practitioners to complete in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in and around Millersburg, OH. This position offers flexible scheduling and a home-based practice that fits your lifestyle.

Qualified NPs will travel to patients’ homes, perform non-invasive assessments, review histories, and provide education while avoiding treatment or prescribing in the home. Training and scheduling support are provided.

Qualifications

• Master's degree in nursing and NP licensure.
• Active NP license and national board certification.
• Current BLS certification and at least 12 months of paid advanced clinical provider experience preferred.

Responsibilities

• Travel to and from patients' homes for HRAs.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and document findings accurately.
• Conduct medical reconciliation to ensure safety and compliance.
• Provide basic patient education on wellness and preventive healthcare.
