Job summary

Signify Health, part of CVS Health, is seeking a Full-time Nurse Practitioner to perform In-Home Health Evaluations for members in their homes. The NP will connect with individuals, review medical history, perform physical evaluations, and coordinate care with health plans.

A clinician uses an iPad and a streamlined workflow during visits and is not required to prescribe medications or order labs. The role includes travel within licensed states, virtual visits as needed, and participation in

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).
