Job summary

Signify Health, part of CVS Health, is seeking a Full-time Nurse Practitioner to perform in-home health evaluations for plan members in their homes. The role includes medication/history review and physical exams, with travel to members across licensed states as needed and virtual visits as necessary.

Qualified candidates are licensed Nurse Practitioners with board certification; multi-state licenses are preferred.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board-certified Nurse Practitioner required.
• Multi-state licenses preferred.

Responsibilities

• Conduct in-home health evaluations for plan members.
• Review medications and medical history, perform physical exams.
• Travel locally to member homes and licensed states as needed.
• Conduct virtual visits as needed.
• Coordinate with Signify Health services (diagnostic & preventive) as indicated.
