Job summary

Signify Health, part of CVS Health, seeks a Full Time Clinician (NP) to provide in-home health evaluations, engaging with people in their own homes and bridging gaps in care.

The visit lasts about 45 minutes, includes medication and medical history review, a physical examination, and, if ordered by the health plan, additional diagnostic tests. Travel locally within licensed states is required; virtual visits may be conducted as needed.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification required for practice.
• Multi-state licenses preferred to enable cross-state work.

Responsibilities

• Provide in-home health evaluations to patients
• Engage with patients to bridge gaps in care
• Travel locally within licensed states as needed
• Conduct virtual visits when required
• Perform diagnostic & preventive services per plan requirements
• Document visits and coordinate with care teams
