Signify Health is seeking a Full Time Clinician (NP or PA) to perform in-home health evaluations for members across various states. You will conduct visits, review medical history, perform physical assessments and coordinate care with health plans, using an iPad and a streamlined clinical workflow.

The role requires active licenses, board certification, and willingness to travel locally and within licensed states. This position offers comprehensive benefits and stable full-time hours.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).

Responsibilities

• Engage with health plan members during in-home visits and address health questions.
• Visit members nationwide to connect them with the right care for their needs.
• Collaborate with Signify Health clinicians and follow the clinical workflow.
• Travel locally and within licensed states; conduct virtual visits when needed.
• Provide diagnostic and preventive services as indicated by health plans.
