Job summary

Signify Health, part of CVS Health, is seeking a Full Time Clinician (NP or PA) to provide in-home health evaluations nationwide. Visits last about 45 minutes, include a medication and medical history review, a physical evaluation, and, if ordered by the person’s health plan, additional diagnostic tests.

You will use an iPad and an intuitive clinical workflow; you are not required to prescribe medicines or order labs.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).

Responsibilities

• Provide In-Home health evaluations using iPad and clinical workflow.
• Perform physical evaluations and review medication and medical history.
• Travel to patients' homes locally and within licensed states.
• Conduct virtual visits as needed.
• Deliver Diagnostic & Preventive Services as indicated.
