Job summary

Optum is seeking a HouseCalls Advanced Practice Clinician (NP/PA) in New Mexico to deliver annual in-home health assessments for Medicare Advantage and other health plan members. You will work independently in the community while collaborating with an interdisciplinary care team to improve member outcomes.

This non-prescribing role focuses on preventive care, education, and care coordination, with a base salary plus incentives.

Qualifications

• Nurse Practitioner with ANCC or AANP certification or Physician Assistant with NCCPA.
• Active, unrestricted state licensure or ability to obtain by start date.
• Active, unrestricted driver's license and reliable transportation.
• Ability to perform in-home assessments in varied home environments.

Responsibilities

• Conduct comprehensive annual in-home health assessments for Medicare Advantage members.
• Travel statewide in New Mexico with expenses covered.
• Collaborate with primary care providers within an interdisciplinary care model.
• Focus on preventive care, education, and care coordination.
