Job summary

Optum is seeking a HouseCalls Advanced Practice Clinician (NP/PA) in New Mexico to perform annual in-home health assessments for Medicare Advantage members. You will work independently in the community within an interdisciplinary care model, focusing on preventive care and education.

Qualifications

• Nurse Practitioner with active ANCC or AANP certification or Physician Assistant with NCCPA certification.
• Active, unrestricted state licensure or ability to obtain by start date.
• Active, unrestricted driver's license and reliable transportation.

Responsibilities

• Conduct comprehensive annual in-home health assessments for Medicare Advantage members.
• Coordinate care and education with primary care providers to close gaps.
• Work independently in communities while supported by an interdisciplinary team.
• Travel statewide in New Mexico with travel expenses covered.
