Job summary

Optum is seeking an Advanced Practice Clinician (NP/PA) to perform in-home health assessments across New Mexico. The role emphasizes preventive care, education, and coordination with primary care providers in an interdisciplinary model.

The position is field-based with travel throughout the state; a license to practice and driving is required. Base salary plus incentives, with student loan repayment or sign-on bonus opportunities available.

Qualifications

• Active NP or PA certification as required
• Active, unrestricted state licensure
• Active driver's license and reliable transportation
• Experience performing in-home assessments preferred

Responsibilities

• Conduct comprehensive annual in-home health assessments for Medicare Advantage members
• Travel statewide in New Mexico; travel expenses covered
• Collaborate with primary care providers to improve member outcomes
• Non-prescribing role focused on preventive care, education and care coordination
