Job summary

Optum Home & Community Care is shaping innovative HouseCalls programs to bring preventive care directly to patients in their homes. As part of the team, you will perform annual in-home assessments for Medicare Advantage and other plans, focusing on education, gap closure, and coordination with PCPs to improve outcomes.

Qualifications

Active, unrestricted licensure in the state you will practice
Active driver's license and reliable transportation
Certification as NP or PA with required board credentials

Responsibilities

Conduct in-home health assessments for Medicare Advantage members
Document diagnoses and care plans
Communicate findings with primary care providers to close gaps in care
Identify urgent or emergent situations and intervene appropriately
Educate patients on diseases, medications, and adherence
Address social determinants of health and arrange referrals
