Signify Health, part of CVS Health, seeks a Per Diem Clinician (NP or PA) to perform in-home health evaluations and bridge gaps in care. You will visit members at home, review medications and medical history, and conduct physical exams using an iPad and clinical workflow.

The role includes travel within licensed states as needed and occasional virtual visits. This part-time, per diem position offers an hourly base rate with incentives, malpractice coverage, supplies, and other perks.

Qualifications

• Active, unrestricted license in coverage area(s).
• Board certification required.
• Multi-state licenses preferred.

Responsibilities

• Visit members at home and perform in-home health evaluations.
• Conduct medication and medical history review, physical evaluation.
• Travel locally and within licensed states as needed.
• Conduct virtual visits as needed.
• Provide Diagnostic & Preventive Services per plan.
