Collage Home and Community Rehabilitation Partners seeks experienced Occupational Therapists to provide truly patient-centered care in Fallbrook, CA and surrounding areas, with PRN positions available. Qualifications

• Bachelor's degree in Occupational Therapy and state licensure required.
• Master's degree in Occupational Therapy preferred.
• Experience in neurologically related disorders preferred.
• Excellent planning, organization, writing and time management skills required.

Responsibilities

• Evaluate and treat impairments including upper extremity function, ADLs and IADLs, visual and perceptual skills, cognitive needs, adaptive equipment needs, assistive technology needs.
• Provide patient-centered home-based therapy in patients' homes and communities.
• Collaborate with interdisciplinary teams to build integrated treatment plans.
