Job summary

Collage Home and Community Rehabilitation Partners seeks experienced Occupational Therapists to join our team for PRN positions, delivering patient-centered care in Spring Valley, CA and surrounding areas. Therapists work within patients' homes and communities, creating integrated treatment plans with our interdisciplinary teams.

Qualifications

• Bachelor's degree in Occupational Therapy and state licensure required.
• Master's degree in Occupational Therapy preferred.
• Experience in the treatment of neurologically related disorders strongly preferred.
• Excellent planning, organization, writing and time management skills are required.
• Ability to communicate with families, professionals, and co-workers is a must.

Responsibilities

• Evaluation and treatment of impairments including upper extremity function, ADLs and IADLs, visual and perceptual skills, cognitive needs, adaptive equipment needs, assistive technology needs.
• Collaborate with interdisciplinary teams to develop integrated, patient-centered treatment plans.
• Support patients in home and community settings to maximize independence and functional goals.
