Qualifications

• ACLS;BLS;PALS;EMT-P Indiana Paramedic license required.
• EVOC or its equivalency within initial probationary period.
• NIMS 100, 200, 700, 800 within probationary period.

Responsibilities

• Provide patient-centered care in home and community settings.
• Perform in-home assessment, vital-sign and symptom monitoring.
• Conduct medication reconciliation and adherence support.
• Deliver patient education and coordinate care with teams.
• Identify barriers and social needs, escalate concerns per protocols.
