Job summary

Responsibilities

• Support the Mobile Integrated Health (MIH) Program and GROW Initiative by providing community-based maternal and infant health services.
• Perform intake activities, screenings, and follow-up visits.
• Assist patients with prenatal, postpartum, pediatric, primary care, and specialty services.
• Connect families to community resources and healthcare providers.
• Coordinate referrals and follow-up to ensure successful connections to services.
• Support transportation and social needs barriers impacting healthcare access.
