Job summary

Reid Health seeks a Community Paramedic to deliver primary care and advanced life support within the Mobile Integrated Health program. You will evaluate, treat, and coordinate with primary care providers to extend access in underserved populations.

Ideal candidate has completion of a paramedic program and 3–5 years’ busy 911 experience, with Indiana paramedic licensure and BLS/ACLS/PALS certifications; grant-funded role with ongoing program evaluation.

Qualifications

• Successful completion of a paramedic program.
• 3–5 years of experience in a busy 911 system preferred.
• Current Indiana Paramedic License strongly preferred; other state licenses considered.

Responsibilities

• Provide primary care and/or advanced life support, evaluate and treat acute illness and injuries.
• Coordinate post-visit follow-ups with physicians and care teams.
• Educate patients on disease management, prevention, and wellness topics.
• Conduct home safety assessments and monitor patient conditions.
• Document findings and report to Medical Director and PCPs per protocol.
• Assist with scheduling and referrals for continued care.
