Job summary

UnitedHealth Group is seeking a Home and Community Based Services Licensed Practical Nurse to deliver direct nursing care in patients' homes and document care in the EMR. You will coordinate other care activities and work under supervision to ensure high-quality patient outcomes.

Required qualifications include current LPN licensure and a valid driver's license; preferred is home care experience and flexibility to meet patient needs.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and vehicle insurance.
• Ability to function in any home situation regardless of client background.
• Home care experience preferred.
• Able to work flexible hours to meet client needs.
• Maintain good communication and organizational skills.

Responsibilities

• Provide direct nursing care as prescribed by the physician in patients' homes.
• Record clinical notes and progress in the electronic medical record.
• Coordinate other patient care activities with the health care team.
