Job summary

The ideal candidate holds an active LPN license, a valid driver's license, and CPR certification (or willing to obtain within 90 days). Home health experience preferred, with strong communication and organizational skills.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Driver's license and access to dependable vehicle or public transportation.
• CPR certification preferred or able to obtain within 90 days of hire.
• Home health experience is a plus.

Responsibilities

• Provide high-quality clinical services per the plan of care and in coordination with the health care team.
• Document patient visits thoroughly in the EMR per regulatory requirements.
