Job summary

UnitedHealth Group is seeking a Licensed Practical Nurse (LPN) to provide high-quality, in-home health services under the plan of care and in collaboration with the care team. You will document visits, follow infection control protocols, and participate in care coordination to address patients’ needs in varied home settings.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred CPR certification or ability to complete within 90 days of hire.
• Home care experience.
• Ability to manage multiple tasks simultaneously.
• Able to work independently.
• Good communication, writing, and organizational skills.
• Ability to work flexible hours as required to meet identified client needs.

Responsibilities

• Provides high quality clinical services within the scope of practice, in accordance with the plan of care and in coordination with other health care team members, following infection control procedures.
• Documents patient visits thoroughly in the electronic medical record.
• Participates in care coordination, communicating patient needs and changes in condition to the team.
• Function effectively in diverse home environments regardless of client demographics.
